i have strange issue,i m connected to my acs using browser for mangment purpose , i m entering the correct username and password but i receive failed to login,why is that?how to fix this issue

Most likely it is not the correct username and password :-)
Either someone changed it or it got reset for some reason.
which acs version ? did you try password recovery ?

I Though the same like in the beginning , I changed to Cisco/Cisco, but also failed to login

Hi,
For testing purpose, you can try login with the Admin username and password you created on the ACS.

cisco/cisco also acts as admin account,i grant it all privileges,but faulted to login
