Core issue
The breakpoint means an attempt to execute BREAK instruction.
This problem is caused when an internal 32 bit counter overflows and wraps around to 0. This counter is not related to user traffic. Typically, this counter reaches the maximum and wraps around in approximately 40 to 50 weeks of continuous uptime.
This is seen only in switches that run these Cisco IOS software releases:
Cisco IOS Software Release 12.2(17d)SXB1
Cisco IOS Software Release 12.2(18)SXD
Cisco IOS Software Release 12.2(18)SXE
Cisco IOS Software Release 12.2(18)SXF
This issue is documented in Cisco bug ID CSCsb98702.
Resolution
Issue the remote command switch test ltl profile stop command in order to enable mode command to disable ltl profiling. This command needs to be issued after each reload. This command also needs to run on the new active after switchover. This command has no side effect on the system.
In order to confirm that the workaround is in place, issue the remote command switch test ltl profile show enable mode command.
The ltl profile status must be Off in order to verify that the workaround was applied successfully.
