Problems with upgrading IOS using Ciscoworks

Hi,

I am having this strange problem while attempting to upgrade IOS for Cisco 3745 Router using Ciscoworks .

I get the following error message.

----------------------------------------

Fetching image from Image Library (for System Software Upgrade):

c3745-advipservicesk9-mz.123-16.bin --> d:\PROGRA~2\cscopx\temp\s3b0.1

Image retrieved from library.

Loading image file to flash device : c3745-advipservicesk9-mz.123-16.bin --> flash:c3745-advipservicesk9-mz.123-16.bin

The rcp transfer has failed. Trying tftp protocol now ...

Error loading image to Flash.

Error Info Code: '4159'

Message: 'Device Error: The copy to/from flash operation failed.'

Action/Reason:

'Probable Cause: Flash copy failed with return code of

copyUnknownFailure.

Recommended Action: Retry the operation. If the problem

persists, contact your technical support representative for

assistance.'

Device has been unlocked from exclusive access.

Image Upgrade Failed

----------------------------------------

The image that I am trying to copy to flash is about 21 MB.

I am able to pull ( backup) image from the same device. The existing image size is 17 MB.

I know there was (or maybe is ) a limilation in Cisco TFTP for copying a image greater than 16 mb but I do not think this to be problem as existing image is being backed up.

What should I try next ( RCP is disabled on CiscoWorks) or can this problem be resolved by applying some CW patch ?

My CW details are given below.

CiscoWorks Common Services with SP2 ver 2.2

RME 3.5 with IDU update 11.0
