We have a Unity 2.45 voice only running on NT. It has been running OK for sometime. But now, although it is up and running the icon in the systray says that it is still starting and does not change. The following errors are in the Event Viewer :-

The AvCsMgr service failed to start due to the following error:
The service did not respond to the start or control request in a timely fashion.

Timeout (120000 milliseconds) waiting for service to connect.

SMTP virtual server 1 has finished initializing queues, accepting connections.

Virtual Server 1: C:\Inetpub\Mailroot\Drop will be used for the drop directory.

Virtual Server 1: Routing extension was not specified. Delivery to mailboxes is not available.

Any ideas?

it could be that during a restart of the system the services up stream from AvCsMgr (i.e. all the Exchange services) took too long to start up and the hard coded time out for service startup expired (that would be that error message in the event log). NT 4.0 doesn't have the "retry X times" option for services startup that Win2K does so the service will simply report that it failed to start in this case. That's on possibility, anyway.

You can try stopping the try icon (not Unity) and reloading it to see if perhaps just the try icon is out of whack. Right click on it, select "exit" and the tray icon is unloaded. Then go to the \commserver directory and run the "AvCsTrayStatus.EXE" application which will reload the tray icon (and repoll the services for their running state).

If it still reports that the service is starting instead of running, what do the AVxxx services in the SCM report?
