7941 stuck in fallback mode after working remotely through ASA Phone Proxy

I'm using CCM 6 and an ASA with Phone Proxy. I have recently pulled one of my phones that was configured to go through the ASA remotely back into the office and it will only come up in fallback mode. I have reset the phone multiple times. I have confirmed that the phone has an internal IP address in the correct range for my vlan configuration, it no longer has the alt-tftp server listed and correctly lists the CCM server address in the network configuration. Any idea why it's doing this? I'm stumped.

Found the fix in the wiki, the ctl file needed to be erased:

Deleting the CTL file on the phone

1. Press the "Settings" button

2. Choose "Security Configuration" (might be option 4 or 6, depending upon phone model)

3. Unlock the phone with "* * #"

4. Scroll down to "CTL file" and press "Select"

5. Press the "Erase" button (might have to press "more" first)
