With G723.1 if you choose the non-Annex A version, then you need to turn ON vad to avoid sending silent packets. If you choose the AnnexA version then VAD is embedded and whether or not you turn on vad on the voip dial-peer does not make a difference: silence packets should be embedded. Make sure that both sides are negotiating the correct codec and VAD settings by "incoming called-number ..." on your voip peers.
