Call setup procedure

When I place a call using VoIP, does the Call Manager answer my request with an IP address that my phone dial-up, or does it connect the call and then the 2 phones run point-to-point against each other without the call manager sitting in-between?

Signaling and call setup traffic is directed at the CallManager, but the RTP audio stream goes directly from one phone to the other without passing through the CallManager.
