Call transfers to VM during the conversation

Since upgrading to CCM 4.1.3 we have had intermitant strange call behavior. On one extension a call will transfer the caller to UnityVM in the middle of the conversation. Another oddness has been users unable to answer a call via a 7940, the phone will ring however when the handset is picked up the it will just keep ringing and the answer softkey does not work. I upgraded to 4.1.3 SR2 a few weeks ago and the problems persist. Your help is appreciated.
