CallFwdAll to show originator CallerID

John Does calls from PSTN number 555-5555, to CallManager MGCP gateway with CCM/7960 Line1 555-1212, but the user on Line1 555-1212 has CallFwdAll set to 400-2483 Cell Phone on PSTN.

The 400-2483 gets a CallerID showing 555-5050 which is our Unity Handler extension.

If a 7960 users dials the PSTN their LineX number shows up via CallerID.

Where should I start looking?

There are ways to get around this. However, most CO's will only allow you to send outgoing caller id for your did range.
