The problem you report looks very similar to a couple of problems that were previously identified (CSCdx94955, CSCdy04947, CSCdy06311). However that problem should be fixed in the build you are running. Each of the problems had to do with what happens on subsequent attempts after an attempt is made to authenticate to the CCMUser page with invalid credentials.
I would suggest that you see if restarting the IIS Admin service resolves this problem, and also attempt to reproduce the problem based on the information I just provided, and contact Cisco TAC with the results.
