+1 vote for 'Gateways and Gatekeepers' by David Mallory et al. It's a great resource, both for the exam, and the real world.

Don't forget that there is good information on Cisco's web site too. This:

.. and the docs in the "Related information" links at the bottom of the above page will help to explain some aspects of dial-peers that are not explained as well as they should be in the student guide.

CCO has good docs on H.323 gateway and gatekeeper configuration too..
