Do you have CMWI turned on in the CENTREX for those lines that need messaage waiting?
If the SMDI link is providing 10-digits, do you have the full 10-digit number for each mailbox added as an alternate extension?
For the Centrex users do you have them set up for CENTREX or Call Manager integration?
I've found that a number of times the wrong integration is set in the profile. In addition for the message block under use MWI, be sure to include the area code and prefix otherwise Unity transmits the wrong info to CENTREX and therefore it doesn't work.
