I'm getting tonnes of Warning logs from the source CiscoUnity_LogMgr in my app event log on my Primary Unity svr. I assume these are coming from the Unity Diagnostic Tools where someone has started a log trace.
All of the warnings description mentions each Unity svc then "Unable to add performance counters for X svc (insufficient memory space).
Sounds like you're hitting CSCeb71678 - there's no release notes on that bug since it was reported as fixed prior to release of 4.0(3) - the notes internally indicate there's some sort of low resource condition that gets it into that state and that a restart of the box (not just Unity) can clear it.
That said, you should probably get TAC to open a case on this and attach it to that bug - sounds like they may have missed something in there that's causing such aggressive logging of the error at a minimum.
