I have an issue with one audio. Its not a everyday occurance, but it is becoming an issue. During a conversation to the PSTN the conversation outbound stops. I can hear them, but the can't hear me. Does anyone have any ideas?

You may test the DSPs to see DSPs performance
in the router.
Which IOS and platform your using, its an FXO or another trunk (T1,E1,etc)
Using sh call active voice bri command
and i button in the IP Phone.
To see if we are ok in the IP side or somehere in the network packets are lost.
Check network topology.
Sniffer trace in the IP Phone just to check its not an IP Phone issue.

I agree, this could be a DSP issue. On the router you can issue a "sh voice dsp" and "sh voice call status" to see which DSP is servicing which voice port or channel, then make some test calls to see if you can replicate the problem when a call hits a certain DSP.
I have encountered several voice quality problems including one-way or no-way audio that only occurred during high call volumes, when a call would get to a "middle" channel on a PRI (ie ser1/0:23.15). A reload would probably resolve the problem, but it could be a bug in the image.

I just had a Unity server do the same thing, but turns out the NIC teaming broke on the server... one nic was the static of the server, the other was DHCP... ugly.... reteamed, rebooted and all was fixed with the audio.
Also, double check your VLANs for the both NICS.
