I have CCM 4.1 & CER 1.2. Is there a Cisco/3rd party utility, so that I can produce a CSV file of all my phones from the existing CallManager database?

Well, you can go to Route Plan Report and click View in File in the upper right corner. It includes all phones. It will prompt you to save a file as a CSV.
Hope this is what you are looking for.
