Different message when line is busy as opposed to no answer

Is it possible for a caller to hear one message when the person's line is busy, and a different message when the line does not answer? Or is this a Call Manager issue?

Yes. There is a busy greeting available for each subscriber to record. Unfortunately, this cannot be recorded over the phone. Currently you have to record the busy greeting using the web based AA(for Unity 3.x or lower) or the PCA(for Unity 4.x).

We are looking into enabling the ability to record and change both the busy and the internal greetings through the phone for a later release of the 4.0 product.

So Unity knows that CM forwarded the call as a result of a forward busy and not a forward no answer?

Yes, Unity knows whether CM forwarded the call as a result of busy or no-answer. AvSkinny.TSP will receive different Redirect Reasons from CM if it was forwarded for busy or no-answer. This information gets bubbled up to higher-level Unit y components that figure out which greeting to play.
