Does anybody has Voice Quality problems?

I have some voice-quality problems like echo , absolute silence , word replication ( not echo ) , one-way voice stream break for 2-4 seconds during the conversation , poor voice quality .

But most of these problems do not occur everytime.

Do you know the exact solutions?
