Error. Failed to initiate a consulting call

I occationally get the above error message on my web attendant when I try to forward callers to voicemail ( Active Voice ). At that time I get a fast busy tone on my 7690 phone.

Has anyone experienced a similar error?.

If you are running CCM 3.1 may be you are hitting this bug CSCdv82694. If it is, then the fix is available in CCM 3.2
