Active Directory had an issue where only Domain Admins and local system accounts had rights to view deleted objects container. Unity must be able to see in to that container to automatically remove deleted users. Microsoft fixed the issue and gave us code which was added to the Permissions Wizard. The first version of Unity to ship with the updated Permissions Wizard was 4.0(2).
Im pretty sure that if you re-run the latest Permissions Wizard that those orphaned entries in SQL will get cleared up. You might need to re-synch with AD after words. You can do that from SA -> System -> Configuration -> Settings -> All Objects.
