fxo ports failing to register w/ call mgr

I have a 3660 with 12 FXO ports. We initially setup two fxo ports and everything seemed to work ok. After we added the remaining 10 and duplicated the config on the call manager and the 3660, the initial two still work, but the newly added fxo ports do not register.

If you are using MGCP, have you added dial-peers for each of the 12 ports? If you are using software before 12.1.5, then mgcpapp will need to be in all caps.

dial-peer voice 1 pots

application mgcpapp

port 1/0/0

dial-peer voice 2 pots

application mgcpapp

port 1/0/1

and so on.....

Which slots are the FXO modules in that do not work? Also, which version of IOS is being used on the 3660, and which version of CallManager? There have been a couple of problems relating to this that have been resolved.
