Hold and Transfer not working due to IP Media Streaming Appl service
Call Manager 312c installation with publisher and one subscriber.
Everything was working ok. Transfer and Hold stopped working - fairly certain that no-one made any changes.
Discovered that when IP media streaming application service is stopped on Publisher then the hold/tranfer starts working. Start the service again and hold/transfer stops working (Incidently, stopping or starting this sevice on the Publisher causes the Call Manager service to stop on the Subscriber causing all the phones to reset!)
There is also a ICD server which uses the Publsher as it's resource manager, which I have a feeling is involved with this problem some how.

I'm experiencing the same problem with 3.1.2c . Cisco was not sure what causes the problem but was able to provide a work around. First option is to check off MTP Requied under the gateway config or apply a voice-class codec statement and apply it to your pots dial-peers.

The below trick might come handy when you have to add a new node to a cluster but you don't have or is unsure of the security password for the publisher. This procedure has been around for ages.
1) Login into the CLI of the Publisher.
