Most of the IP Phone loads come in a self extracting format. You will need to run this on the callmanager and it will automatically place this file in the TFTP folder and will also update the Device Defaults page with the new information.
After this, you can reboot one phone and it will automatically upgrade the image.
On a per phone basis, if you have the .bin phone load, you can place that file in the TFTP directory of the callmanager and go to the phone configuration page of the appropriate phone and specify the image name in the "Firmware Load Information (leave blank to use default) - > Phone Load Name" section.
