Information from Outlook to Softphone

HI The Customer has Nt4.0 with exchange 5.5. We have softphone installed on the PC's. Now The customer wants to pull the inforamtion from Outlook into SOftphone and dial the phone number.What are the ways It can be done. Also Customer has 3 remote sites and one central site.They are using Citrix to access the outlook on remote site while at central they use directly. Thanks for any input in advance.
