Integrated Mailbox Configuration issues

The problem occurs only for non ccmadministrator user. All other users who have full access to CCM experience this issue. When I click on the Create mailbox link in CCM admin page, I get "ASP file is not found in the mapping" and a CCM page displays Access Denied. I never get prompted to enter username.

CCM 4.1(3)sr1 Unity 4.0(5).

Digest security was selected.

If I remember correctly, when you first run the Integrated Mailbox setup, you have to set a specific account (usually administrator or ccmadministrator) that's to be used with it.

I see in the Integration Guide that you can set more accounts to be used; see here:
