CCM version is 4.02asr2c.
Attendant console is 1.3(1ES10)
Centralized cluster with 14 remote locations on 512K to 768K Circuits.
Each location has an operator and an attendant console installed and each site handles their own calls.
Intermittently at few locations attendant console refuses to launch and the only resolution to the issue is to uninstall and reinstall.
Also, sometimes inbound calls to the pilot point of few locations gives a fast busy and if we reset the TCD service the issue gets resolved.
Any ideas will be greatly appreciated

Try upgrading to a latest version of Attendant console.

Dont remember any DDTS.
This will involve CCM|SDL|CTI|TCD traces analisys.
So i recommend you to open a TAC case with us to get a better response.

I hate to say this but three TAC cases was opened in the past regarding issues with our ACs and an ES was built specially for us but the issues continues to come back.
Right now we are planning to go to 4.1(3)SR3 and upgrade all of the ACs to the shipping version of CCM hoping that all of the intermittent issues goes away.
