when you deploy new phones they register to CCM automatically, from that phone you call a Directory Number (I gues it was CTI point) and enter the phone number you want that to assign etc... Phones profile information have to be in the system beforehand.
Couple weeks ago I have searched Cisco and came up with an IP Phone deployment scenario like this one. I guess, that web page has been removed, I can not find it again.
