It appears the mgcp gateway will answer and send dialtone. It doesn't forward to call manager.

Dialtone for IP Phone is send by CM....
You should chech registration of mgcp gw in
Application/Call Manager Serviceability ->
Tool/Admin Serviceability tool ->
Devices -> Gateway devices....
If it is registered than OK. If not then
check you gw name (it should be exacly like in "sh ccm".
If it still not work - post you configs here...

If you are talking about a loop start port (FXO) on the MGCP gateway, then you have to fill out the Attendant DN field on the FXO endpoint in CallManager configuration if you want the call to automatically route to an IP phone.
