I have validated Multicast MOH with an MGCP gateway (I think it does not work with H.323). Once you have enabled multicast in CCM config (audio source, moh server and MRG), you just have to configure 'ccm-manager music-on-hold' in the gateway so that it receives the multicast address when a PSTN user is put on hold.
Here is an excerpt from the gateway config (mostly done by CCM) :
