Installing a new dual integration Unity and everything appears to be working except MWI. I can see the MWI message in the Integration Monitor but no change to the light on the Fujitsu phone.
The MWI Code sent for extension 4430 is:
<CR><LF> OP:MWI 0004430!<EOT>
According to the Fujitsu integration docs for the old voicemail system, the MWI on code should be:
"OP:MWI 0004430!/D"
Documentation on integration of Unity with a Fujitsu is scarce.
The PBX is a Fujitsu 9600, Unity 3.1(5)

The documentation is scarce because the integration is not supported. To set your expectations, TAC and Cisco aren't going to support it.
Here is a list of the supported integrations:

The account team got the PM to sign off on this integration. This was supported by ActiveVoice and will be supported by TAC on a case by case basis. There's an acceptance document that the Cisco SE had to complete and attach to the customer contract.
We got a Fujitsu integration file from the PM. We modified the avsmdi.avd file at the recommendation of TAC to remove the leading CR, LF and that seems to have corrected the MWI problem.

The
Control characters aside, both voicemails agree the proper SMDI message is
OP:MWI 0004430!
