Yep it's a bug. I bet you have exactly matched the extension numbers that you want to use personal assistant, so int the PA config you have listed the extension numbers. Well guess what, you can't do that. there is an internal Cisco Bug reference for this.
What you have to do is set up your route points with wild cards e.g 2xx, 3xx, 4xx and set up PA with the same numbers. If you do this then it should work OK. I struggled for about 6 months with exactly the same problem.
The trouble shooting guide for PA gives a whole host of other reasons but this is the biggest problem.
Don't forget to associate the new RPs with the PA user in the global directory.
