Ringing on second line.

Is there a way to get a second extension to actually

give an audible ring if the user is off hook on the primary extension?

This has been a big issue in the field. Currently the one way around it is to disable Call Waiting and setup multiple shared extensions. This solution has worked great for me but creates user confusion with all the extensions on the phone.
