I have multiple lines on some phones, as well as a hunt point being used with multple lines and our Console. Once a call is on the first line, all calls bounce directly to VM and do not roll over to the next line. This happens both with lines configured to roll from one to another, or with a hunt group as mentioned above. We are running CCM 4.1 (3) sr3a and this started at the time we upgraded to this.
There were two different problems resolved by re-configuring the hunt group as well as the line appearances on one of the phones that share those lines. The problem for some reason did not become realized until we did the upgrade...so it was a coincidence.
