I have one user who is unable to retrieve vm through their IP phone. Receiving "system temp unavailable". When I look at their mailbox size on the SA Web page it shows 0 KB . We have had this in the past, where the Doh Prop shows the wrong exchange server. I verified it is the correct server. Any suggestions?

There will be an error (or errors) in the application event log when you hear the failsafe message. Can you take a look and see what it's complaining about?

Was the mailbox moved recently? Exchange(Unity) may be confused as to where the mailbox is at so it can't access the mailbox.

The mailbox has not been moved. I tried deleting and recreating subscriber in Unity, and still shows mailbox size of 0KB
