Transfer SoftKey Unavailale

Incoming call from PSTN, press the transfer key on 7960, dial the DN (extension) where need to transfer to, now have to press transfer key again to complete transfer, but the transfer softkey is not available now. Problem happens intermittently. Running CCM 3.0(11). Phone Load is P003E310.

Sometimes even the incoming call drops when trying to transfer.
