Transfer to ICD doesn't give ringback tone

We have ICD V2.2.3a and Call Manager V3.1.3a SP D.

We have an advertised number which is a second line on a 7960 IP phone which is set to Forward All to the CTI route point number.

If you call the advertised number from the PSTN you hear a very short ring and then silence while the agents phone is ringing.

If you call the advertised number from an IP phone you hear Music On hold while the agents phone is ringing.

In both cases I would have expected to hear ringback tone.

Can anyone explain this? Can we change it so we hear ringback?

Several bugs associated with this, may want to start with CSCdt17793.
