The subscriber template you used to create these subscribers may not have the "first time enrollment" flag set (it's optional). You should be able to manually turn it on for the folks you want to in the SA on the subscriber's profile page (or in bulk using the BulkEdit tool if necessary).
The self enrollment conversation doesn't have anything to do with the CM integration... if you have that flag set and you sign in as that subscriber over the phone, you should be walked through the first time enrollment conversation.
Are you saying you are being signed in as that subscriber at all when you call in from their extension? Or are you being signed in but just not getting the first time enrollment conversation (i.e. you get right to the message stack)?
If you enter the mailbox manually (i.e. at the opening greeting press * then enter the ID of the box) you can log in and get the self enrollment conversation, right? Sounds like we're not getting the calling extension properly from the switch and so we don't automatically route you to your mailbox.
