Cisco Unity 4.0(1) has been released on CCO and can be found at http://www.cisco.com/kobayashi/sw-center/sw-voice.shtml.

Can the VMO from 4.0.1 be used with older versions of Unity (specifically 3.1.5)?
I'm specifically interested due to the ability to deploy the new VMO via SMS or Group Policy.

Yeah it should be fine. The code is basically the same.

Just to follow up on this - the fine folks in QA passed on that they had, indeed, tested this configuraiton and it's supported. Here's a link to the official support matrix for client software vs. back end Unity version:

One question - when I launch the upgrade tool it gives me the option for
UNITY40-SL16-VERUP Unity, UM or VM version update to 4.0 - 16 session license 1
UNITY40-SL32-VERUP Unity, UM or VM version update to 4.0 - 32 session license
I'm confused why it offers two - are those both necessary to order?

I have a new Unity server, do I need to install the 3.x version then upgrade, or can I just install the 4.01 fresh?

You can install 4.0 fresh baring that you have a license.

I processed my key upgrade request at the site listed - but it's currently manual and it gave me an email address of unitykeyexchange@external.cisco.com which bounced - just on a whim I tried unitykeyexchange@cisco.com and it didn't bounce - but I'm not sure that means it's right - can someone confirm if this was the right approach and it's just going to be a delay for a few days because of the demand?
The address with the @external.cisco.com address was/is https://www.cisco.com/pcgi-bin/Software/FormManager/formgenerator.pl?pid=321&fid=1501
