I am installing the primary 4.0.1 server for failover. The Unity install runs and finishes and asks me to reboot. When I reboot the server comes up with no AV services installed, things like the HKLM\Software\Active Voice\DirectoryConnectors\DirSynchGlobalCatalog\1.00\Directory aren't even showing up in the registry. Any ideas?

How are you launching setup? You should launch it from Setup.exe in the root of the CD. Setup.exe should launch Cisco Unity Installation and Configuration Assistant which launches SvcWiz.exe and configuration licensing. CUInstall.exe isn't what you want to install from.
