Info for both of these is right at the top of your dbWalker output log. This could be a bogus error - in Unith 4.0(1) they changed the owner/recipient links of templates to be "NULL" instead of linking to the example admin account as they did in the past - dbWalker was not updated to change this check until post release - if you download an updated version off of www.CiscoUnityTools.com that's fixed.
If you're running on an older 3.1(x) version of Unity there's a different issue at play.
