I have several 7960's with 1 each 7914 attached that have been working fine. I went to add a second 7914, first unplugging phone and 7914, then changing config to 'type 7960 addon 2 7914', then plugged the phone back in.
Now both 7914's are blank and all red lights, in spite of multiple phone resets and power cycling and combinations of both. If I change the config back to 'addon 1' and unplug the second 7914, everything is good. When I change the config back to 'addon 2' but second 7914 is still unplugged, first one is back to all red.
Any suggestions would be appreciated.

Yeah, this is the correct syntax.
type 7960 addon 1 7914 2 7914

I would verify that you have the phone loads. Do you have the 7914 load specified under the telephony-service?
(7960)
tftp-server flash:P00308000400.sbn
(7914)
tftp-server flash:S00105000100.sbn
telephony-service
load 7960-7940 P00308000400
load 7914 S00105000100
ephone 1
mac-address 001A.6D0E.83CE
type 7960 addon 1 7914 2 7914
You can run the show ephone command to verify the phone and modules have the right load.
show ephone phone-load

So the proper syntax should be "type 7960 addon 1 7914 2 7914" as you have stated?
The syntax I am using is "type 7960 addon 2 7914"...

Hi Michael,
Craig is correct here! 5 points for this nice answer Craig. There is a good reference in this doc;
type phone-type [addon 1 module-type [2 module-type]]
Example:
Router(config-ephone)# type 7960 addon 1 7914 2 7914
