VG200 w FXS disconnecting on hook flash

Using a VG200 to connect to an analog phone bridge. When a call is placed to the bridge it is answered and put on hold properly. When the call is sent to the conference it is disconnected 50% of the time. Reading some specs on the phone bridge when the call is transferred to the conference the FXS port is seeing a hook flash. This is when the call is disconnected and the ip phone gets a fast busy.

Any thoughts?
