VMO v. 2.4.6, Unity 2.4.6, Win2K Sp2, outlook <br>VMO was installed and working. It has quit. I've tried uninstalling, running VMOCleaner, reinstalling several times but still get the following:<br>"The custom form could not be opened. Outlook will use an outlook form instead. We have had the same experience. Exchange server offbox and there were no event log warnings on the unity server or the exchange server.

When this happens, it's either an integration issue, or as haschulz metioned, a problem logging into Exchange. If it's an integration issue, I would only expect it with certain types of integrations. You guys are using serial, right?
If the system is at 2.4.6.135 and up, there's a registry key edit you can make to have Unity write an EventLog warning when it gets a call without call information (as if there were no integration information at all, or a timing issue with the integration information). This key is...
HKLM\Software\Activevoice\MIU\1.0\Intialization\Log Event Warnings for Calls Without CallInfo = DWORD
Setting that to "1" will make those warnings appear. This may help to correlate the VMO issue with a PBX integration issue.
