Our Unity 4.0.3 UM with an Avaya G3R, PBXLink integration, will periodically send voice messages to the incorrect e-mail account, i.e. message is left for John, I must assume that the caller hears John's greeting (maybe I am wrong, mis-integration is hardly unheard of), but the message ends up in Mary's inbox.
This can create some sticky problems in certain circumstances. Of course it does not happen very often, so it is hard to diagnose.
I guess I am hoping that somebody on the forum has seen this behavior before and has come up with a fix.
As in all cases of "proving the negative" I really won't know that it is fixed until after a few months go by and it does not happen again....
