It depends on how you are routing between the VLANs. If you have high speed routing between the VLANs then I wouldn't be concerned with where you place Unity and Exchange. If you don't have high speed routing (like you're trunking to a 26XX) then I would place Exchange and Exchange in the same VLAN.
Phones and CCM should be in the same VLAN for sure.
