I have read the documentation on this particular blade for the 4006 and I understand that ports 1 and 2 are the ONLY uplink ports that can provide dedicated 1Gbps throughput. The other 16 ports are segmented into groups of 4 that are oversubscribed ports; which can also be port-channeled, etc....
My question is do the ports that are grouped in 4's pair up vertically or horizontally?

I think you may be incorrect there. Read the following section Figure. 5-1 it seems to say that 3,5,7,9 are a group. Read what it says about server D in the example and you will see that in order for the example they give to make sense is if ports 11,13,15,17 are in a group; thus the groups of 4 ports after port 1 & 2 of course go left to right. I hope I didn't make this too confusing but take a look @ the article and assess it.
