3660, NM-2W disabled deactivated

When installing an NM-2W with two WIC-1DSU-T1's in it into one of our 3660's running IOS 12.2.12a the card does not initialize and a show tech provides me with the following:

Is this an IOS mismatch or a hardware issue?

I would suggest re-seating the card and reloading the router. Look at the bootup sequence to see if you are getting any error messages. Another thing to try is to put the NM in a different slot or trying the NM without the WICs in it.
