3661's fastethernet 0/0 can't be up

if I no shutdown the fastethernet,it tells me:

amdp2_fe-1-initfail:CSR0=0X0000

and I add a NM-1E module,it can be up,but can't ping,I unplug the cable,the ethernet3/0 is also up!

why?the ios is c3660-is-mz.123-4,t.bin

Documentation says that if shut/no shut doesn't work you should contact your Cisco representative.
