4148RJ-45 card

On three separate occasions, one of the WS-X4148-RJ cards on a 4506 chassis goes into "inactive" state. Any thing connected on this module stops communicating. When I accomplish the command "show port status", instead of the usual connected/not connected I see "INACTIVE". Only until this module is issued the command "RESET", do all devices communicate once again. Anyone seem this particular problem ???

Can you paste the output of sh mod and sh port status(if you have saved a copy?

A port would normally show inactive state if it does not get VLAN info? Is this switch a VTP client? When the problem happened did the trunk to the VTP server go down?
