What causes the SFP gigabit ports on a Sup V to be inactive? 2 of my 4 ports on each sup are inactive and will not connect.

I believe you have redundant WS-X4516-10GE Supervisors in the chassis. If so, you cannot have all 4 SFP uplinks to be active at the same time.
