Will uplink fast work with 802.1q ?

Yes it will and so will backbonefast. It is disabled by default. The command to enable it is:

switch>(enable) set spantree uplinkfast enable

Does uplinkfast get set on the access switch and the distribution switch. Both ends of the trunk, or only on the access switch?

As far as I can remember, Uplink Fast will only work on a switch that has a Bolcking port, releasing it without negotiating the topology change with the other switches.

The point is that you´ll set uplink fast on a switch that have one blocking port, and one designated port (forwarding), tipically an access switch (just tipically.. but needn´t be this way). The opposite end of your blocking link doesn´t need any configuration, since it´ll work normally.
